We are looking for a compassionate and dedicated Registered Nurse Case Manager to join our ‘ohana at Community Empowerment Resources. We are a 501 (c)(3) nonprofit case management agency serving the mentally ill population of O’ahu.
About Community Empowerment Resources
Our mission is to assist our clients comfortably and successfully integrate into the community using the clinically-based SAMHSA Recovery Model as it relates to mental health. This is an opportunity to provide life management services and health care assistance to individuals living with a diagnosed mental illness.
Community Empowerment Resources formed in 2007 with the single goal of helping our fellow brothers and sisters of O’ahu manage their mental health while living happy, healthy, and fulfilling lives. Many of our clients struggle with substance abuse, houselessness, and challenges with the law. We provide the guidance and support needed to navigate through the various social service programs on the island that are designed to nurture, aid, and empower those living with a mental illness.
About the Registered Nurse Comunity Based Case Manager Position
Our RN CBCM will provide our clients with medical assessments, basic health care advisement, wellness education, and general coordination of medical needs including psychotropic and medical medication administration. Our RN Case Manager will have an active role in the provision of healthcare management and rehabilitation services and serve as an advocate for our client's needs. This includes but is not limited to coordinating treatment with PCP/Prescriber/NP/PA/RN or licensed personnel, seeking prescription clarification and advisement, completing required paperwork and file maintenance including DHS forms, and injection administration.
Our RN Case Manager will be uniquely positioned within the fabric of our nonprofit as an essential member of our ‘ohana at Community Empowerment Resources. This role will act as a vessel designed to cultivate a safe space for our clients where they can seek the proper healthcare services needed that are reflected in the national standards of care. This position reports directly to the Community Based Case Management Team Leader, with general oversight by the Psychiatrist, and/or the Medical Director, with administrative oversight by the COO.
Required Qualifications
- License to practice in the State of Hawai’i with a minimum of 2 years of experience
- Certified in CPR, First Aid, Blood Borne Pathogens and CPI
(May obtain within first 90 days after hire) - Be willing to work flexible hours due to the nature of crisis/on-call/emergency situations
- Possess a valid Hawai’i State Driver’s License with a clean driving record Must provide proof of current insurance and written vehicle safety check at the time of hire
- Occasionally lift up to 50 lbs

Pay
Starting pay is $35 to $40 per hour depending upon experience and degrees. Performance-based raises at least annually and significant raises over three to five years.
