Home Care Intake Coordinator (Far Rockaway)

Rockaway Home Care is a Far Rockaway based Home Care Agency. We are hiring Intake Coordinators. The Intake Coordinator is responsible for expediting the paperwork process though Intake. Candidates should be able to multitask and work in fast pace environment. Bilingual required.

Responsibilities:
  • Responsible for non-clinical Intake paperwork.
  • Review referrals and verify all physician information.
  • Data Entry
  • Schedule assessments and complete necessary paperwork.
  • Initiate contact to gather required clinical and demographic data from patient and sources.
  • Timely communication with sales team and supervisors
  • Maintain effective relations with all agency contacts

Qualifications:
  • Must be Organized and detail oriented.
  • Excellent communication skills.
  • Ability to work independently and make decisions.
  • Must have Home Care experience
  • Bilingual English / Spanish
