Below is an itemized listing of all of the tasks involved in the performance of this position:
Communication/Information Sharing
Attendance at Registered Staff Meetings and Care plan rounds
Admission and annual care reviews with resident, family, and physician
Ensure that e-mail and memos are communicated to unit staff
Communicates effectively to all team members and ensures continuity of care by reporting and updating daily 24hour unit report
Timely completion of RAI-MDS assessments and RAPS
Development and modification of the care plans in response to residents’ change in status
Liaises with other health care personnel, agencies, and families
Assists in the investigation, documentation, resolution and reporting of complaints and incidents (i.e. suspected abuse)
Maintains good relationships with Medical/Support/Home, staff, residents, families and the general public
Leadership
Set personal objectives and unit goals in collaboration with the Assistant Director of Care and/or Director Of Nursing
Assists with the orientation of family, residents, and staff on the unit
Monitors performance and completes performance evaluation for the PSW’s
Directs, assigns and evaluates the work of the PSW’s
Maybe in charge of Home as required )
Utilizes leadership skills to identify problems and problem-solving on the unit
Monitors that care delivered is consistent with the resident Bill of Rights.
Quality/Risk Management
Coordinates all quality improvement activities on the unit
On-going monitoring and evaluation of the weight program
Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
Required to participate in the improvement of the indicators and achieve satisfactory results.
Financial
Orders and ensures nursing supplies and equipment are properly and efficiently used
Ensures personal belongings are properly stored and labelled
Professional
Good standing with the College of Nurses
Adhering to the standards set out by the College of Nurses and the Home’s policy and procedure
Ensure Ministry Of Health and Long-Term Care Homes Act and Regulation 79/10 are met
Current CPR Training
Growth and development
Unit Level Activities
Conducts unit rounds at the beginning and end of the shift.
Provides Nursing Management with documentation required for Home’s activities and analyses data (i.e. falls, safety, infection control, unit summaries)
Promotes a safe and secure environment in which the resident resides.
Committees
Participate in the Home’s team committees
Ensures unit minutes are documented and filed appropriately
Chairs unit meeting (if applicable)
Clinical Coordination
Administers medications and treatments in compliance with the policies of Pharmacy provider, the Home and the Standard of the College of Nurses. Documents and reports all errors and omissions
Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of the Home
Participates in the admission, discharge, transfer of the resident to/from the unit
Ensuring completion of documentation records by PSW
Coordinates unit staff activities in event of an emergency
In charge and directs clinical practice on the unit.
Assesses changes in health status and reports changes to Substitute Decision Maker (SDM) and interdisciplinary team.
Updates plan of care as needed
Completes assessments (RAI-MDS and other assessments) and develops a plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team.
Compliance with Standards
Maintains environment, safety on assigned unit and reports to the appropriate Department Head any hazards, which require attention.
Compliance with College of Nurses Standards, Long-Term Care Homes Act and Regulation 79/10 and the Home’s policy and procedures
Profession Values and Ethics
Protects the rights of the residents, families in matters such as confidentiality, privacy, beliefs, and values.
Ensures that ethical guidelines are met in accordance with the standards set out by the College of Nurses
Reporting of ethical issues to the Assistant Director of Care /Director of Nursing
Responsibilities to Health and Safety
Is aware of employee’s responsibilities and follows all Health and Safety policies and procedures.
Works safely to reduce the risk of injury to self, co-workers, and residents
Is alert to and properly reports all actual or potential hazardous situations to immediate supervisor.
Is aware of WHMIS regulated products and takes safety precautions accordingly
Does not operate or use faulty equipment
Promptly reports personal injury to immediate supervisor and seeks first aid as needed
Participates in fire safety demonstrations and fire drills and knows the Home’s emergency plan
Maintains first aid box and renders first aid as required
Has a good understanding of the “Worker’s Responsibilities” under the Occupational Health and Safety Act.
Is aware of how and when to use personal protective equipment (PPE).
Attend all mandatory health and safety training sessions
Report hazards that you are aware of to your Supervisor and the JHSC
Understand and follow all of the Home’s health and safety policies and procedures
General
