A day in the role of a Social Worker at Berkshire Care Centre.
As a Social Worker at Berkshire Care Centre, you will be given high expectations, as well as an expectation to continue personal growth and professional growth. In this position, you will be gifted with a rewarding experience every day in the form of being able to see the exact impact you place on our residents.
As a Social Worker in Long Term Care, you will be part of our Interdisciplinary team, and you will report directly to the Executive Director. You will work 75 hours biweekly and flexibility will be required. This is a unionized position.
This position is responsible for, but not limited to:
- Provide a complete resident admission process including tours, admission, assessment, discharge and coordinates the interdisciplinary team conferences.
- Act as liaison, as appropriate, with families. Provide support and counseling sessions for residents and families as identified, i.e. Orientation adjustment programs, health tracking programs, etc. Evaluate program’s effectiveness.
- Maintain established department plans, policies, procedures to meet regulatory standards including quality assurance.
- Perform MoCA/cognitive assessments as ordered by physician.
- Maintain all required records, reports, statistics, etc. for both residents and department in accordance with policies, procedures and legislative requirements.
- Establish and maintain communications with other disciplines, residents and families to ensure resident needs are being met.
- Act in an advocacy role as appropriate.
- Liaise and consult with government officials, company resources and professionals in relation to resident care needs and department activities.
- Assist residents with discharge planning and application for various available funding and services upon discharge.
Qualifications

- Bachelor’s degree or diploma in Social Work/Social Service Work from an accredited University/College
- Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Minimum of 2 years of experience in Social Work with seniors in Long-Term Care (LTC) setting mandatory
- Knowledge of applicable legislation and individual counseling
- Knowledge of Occupational Health & Safety practices, principles and legislation
- Computer literacy
- Must have a clear vulnerable sector screen police check, no older than 6 months, 2-step TB test results less than 6 months old, evidence of flu shot, must be able to provide three work references.
