Nursing Restorative Lead

We are looking for a team Nursing Restorative Lead for a 1 year contract at Cedarvale Terrace LTC

Cedarvale Terrace is located in Toronto, steps from Saint Clair West subway station. We are recruiting for dedicated, hard working, energetic, and flexible staff that want to make a positive difference in providing care to seniors in their Home.

Responsibilities

  • Conducts interdisciplinary care plan rounds when necessary and documentation of care plan rounds in PointClickCare.
  • In conjunction with the Unit Supervisor, completes initial assessments by reviewing the MDS coding/triggers and RAPs for participation in Nursing Restorative Programs and collaborates with the inter-disciplinary team.
  • In collaboration with the Unit Supervisor and the interdisciplinary team, completes re-assessments for changes in status (significant change), re-admissions from hospital for participation in Nursing Restorative Programs
  • Complete documentation and accurate records as required including: participation in tracking monthly statistics, Intake documentation, Quarterly documentation, Multidisciplinary documentation (ACR), Special Care Review Documentation
  • Works under the supervision and directions of the Nursing Restorative Lead
  • Implement Nursing Restorative programs that meet the physical needs of the residents including: ADLs, bed mobility, transfers, toileting, walking, etc.)
  • Process and monitor all referrals and modify programs when necessary to ensure individual needs are met in collaboration and supervision with the inter-disciplinary team
  • Work in conjunction with the Nursing Restorative Lead, Unit Supervisor, Nurse Managers, Physiotherapist, Rehabilitation Therapist, Occupational Therapist, PSWs, etc.
  • Monitor PSWs’ techniques and practices when nursing restorative care is being provided
  • Educates PSWs on Nursing Restorative Philosophy and techniques, and acts as a resource for PSWs
  • Share relevant nursing restorative information to team members to assist them in their daily responsibilities 
  • Develop external partnership with service providers and other LTC facilities.
  • Required to understand the nature and meaning of quality indicators used by the Home 

Professional Responsibilities:

  • Current certificate with recognized institution 
  • Experience working in a long term care setting
  • Committed to promoting residents’ well being
  • Ability to develop, implement and evaluate programs
  • Demonstrated leadership, communication and interpersonal skills
  • Understands adult learning principles
  • Computer literate
  • CPR and First Aid qualifications
  • Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1) year of being hired, RAPs and Care planning
