A day in the role of a Quality Improvement Coordinator at Cooksville Care Centre.
As a Quality Improvement Coordinator at Cooksville Care Centre, you will be given high expectations, as well as an expectation to continue personal growth and professional growth. In this position, you will be gifted with a rewarding experience every day in the form of being able to see the exact impact you place on our residents.
As a Quality Improvement Coordinator, you will be part of our Quality Team, and you will report directly to the Executive Director. You will work 75 hours biweekly and some flexibility will be required.
The Quality Improvement Coordinator takes the lead in the internal Quality Improvement program and is very familiar with all Residents First Initiatives, co-chairs the Quality Team Meetings and coordinates the Quality and Risk Management Program and Resident Quality Inspection process (RQI) by fostering a culture of continuous quality improvement with a focus and emphasis on safety.
This position is responsible for but not limited to:
· In collaboration with the Leadership Team, develops and submits the Annual Health Quality Ontario (HQO) Quality Improvement plan and Progress report for the Home
· Coordinates the Quality Improvement program within the home and schedules and distributes audits
· Ensures audits and annual evaluations of programs are completed
· Trends and analyzes audit and evaluation outcomes
· Collaborates with the Quality Team members and Executive Director (ED) to schedule quality improvement activities and Quality Team meetings
· Leads the internal Quality Team Meetings
· Attends and reports to the corporate Quality Council
· Coordinates the RQI process in the home. Schedule and assigns Abaqis surveys around the RAI-MDS schedule
· Trains new staff on the RQI process and the Abaqis interview questions and surveys.
· Pulls reports from the Abaqis program and analyzes data with department heads and puts quality initiatives in place accordingly
· Together with the Manager of Clinical Informatics or RAI-C pulls CIHI reports and analyzes data
· Ensures that all quality management and risk management related activities are reviewed, discussed and follow up actions identified as necessary e.g. QIA indicators and exceptions, audits, inspections etc.
· Reviews data from Quality improvement activities, conduct analysis and identify and report trends to the interdisciplinary team, Residents' and Family Council
· Provides education on definitions of Quality and Risk and ensures that Quality and Safety is incorporated into agendas of all team meetings
· Works together with the Accreditation Coordinator to ensure all quality and safety standards for CARF or Accreditation Canada are being addressed.
· Prepares and submits reports on risk and quality improvements and initiatives to the Quality Team and Quality Council as required
· Contributes to the annual report by completing areas that relate to risk and quality
· Participates in Ethics Case Review
Qualifications

· Comprehensive knowledge of health care practices, trends, and issues in Long-term care (LTC) and impacts on other health care sectors/community at large
· Proven management experience in LTC
· Excellent written and oral communication skills
· Excellent knowledge of Quality/Risk Management philosophies and programs
· Master trained in Abaqis/Resident Quality Inspection process
· Good understanding about the Resident Assessment Instrument Minimum Data Set (RAI-MDS) 2.0 process and Canadian Institute for Health Information’s (CIHI) indicators
· Working knowledge of relevant computer applications and applicable legislation
· Positive working attitude
· Strong interpersonal and team building skills
· Must have a clear vulnerable sector screen police check no older than 6 months, 2-step TB test results less than 6 months old, must be able to provide three work references
