Peer Recovery Supporter Job Description

Essential Functions:
· Provide feedback and client and treatment team about recommendations for Individual Treatment Plan and implement individual treatment plan collaboratively with each client and the treatment team. Treatment plans include problems, goals, and measurable objectives. Work as a member of an interdisciplinary team with Hope Village to improve integrated care
· Coordinate programs outside at other locations, agencies, institutions providing peer support and coordinating referrals with accurate outcomes data.
· Provide interventions, information, community referrals, screening and emergency services to clients of Hope Village.
· Make recommendations for therapy and services or medication as indicated for clients to the treatment team.
· Treatment functions with a trauma informed, client centered approach to care and from a peer perspective.
· The ability to know and understand that there are multiple pathways to recovery and willingness to explore this from a client center approach.
· Continually monitor and evaluate each client’s progression and provide regular feedback to the treatment team.
· Coordinate care with all other professionals involved in client's case from Hope Village or other outside agencies. Refer client to the treatment team for crisis intervention to patients of Hope Village as required.
· Provide support and individual skills training to clients as indicated in treatment plan from a peer perspective
· Working knowledge of DSM 5 and ASAM level of care, as well as mental health issues as it relates to providing peer support services to clients at Hope Village.
· Contact supervisor or designated back-up for clarifications of procedures or clinical assistance as needed.
· Maintain a network of professional relationships with relevant service providers in Portage County.
· Work closely in collaboration with clients to engage them in the decision-making process and help them understand the benefits of assessment and accessing the appropriate level of care as indicated in the assessment and treatment plan.
· Complete paperwork requirements and maintain quality, up-to-date clinical records, including billing in client records within 24 hours. Ensures that all records and documentation meet HIPAA, Medicaid, OMHAS and CARF standards. Fully understands and applies all Ohio Medicaid rules for treatment, documentation and billing.
· Participates in regular supervision, team meetings, staff in services and ongoing training at Hope Village and also maintains all required hours for license renewal.
· Maintains professionalism at all times. Engages in a team capacity and will take on additional duties as needed or assigned.
· Meets productivity standards.
Qualifications:
· GED or higher. Must possess certification from the State of Ohio to be a Peer Supporter.
· Knowledge and understanding of documentation per Medicaid standards in proper implementation.
· Ability to keep up and work in a fast-paced environment.
· 1-year experience in a treatment setting preferred.
