Facility Notes: CLINICAL DUTIES|-- * Monitors nutrition screening on newly admitted patients to identify those at nutrition risk.- * Performs thorough nutrition assessments using the Nutrition Care Process. Identifies areas for nutrition intervention and provides accurate documentation. Communicates any recommendations to physician and staff.- * Provides follow-up nutrition assessments as needed and per policy.- * Enters any nutritionals or oral supplements into dietary management software as is appropriate for the facility.- * Analyzes and documents calorie counts.- * Provides nutrition support assessments and recommendations.- * Communicates freely with all nursing staff, pharmacy staff, speech therapy and all other team members involved in the patient’s care.- * Offers diet education for all patients with an admitting diagnosis of CHF and Diabetes. Perform other diet education per physician consult request.- * Actively participates in quality improvement, policy & procedure revisions, and other administrative duties.--Performs other duties as assigned or directed.--NUTRITION EDUCATOR DUTIES-- * Performs medical nutrition therapy (MNT) sessions or nutrition counseling/education for all outpatient referrals per CMS guidelines and according to the Scope of Practice.- * Documents outpatient MNT or nutrition counseling/education session and communicate results to referring office.- * Enters billing information for outpatient MNT or nutrition counseling/education on each patient.- * Attends formal Pritikin Intensive Cardiac Rehab (ICR) training, and maintains appropriate standing in regards to the knowledge and understanding of the content contained in the Pritikin program material.- * Performs one on one Registered Dietician counseling visits with each new phase II, Pritikin Intensive Cardiac Rehab patient.- * Sets applicable goals with each patient, and follows up on those goals throughout their duration of the program.- * Leads Pritikin, nutrition specific educational workshops/classes, according to the Cardiac Rehab class schedule.- * Coordinates, and leads weekly Pritikin cooking schools (one day per week), in which education is given to the patients, and the food items are prepared during the class.- * Provide community and employee nutrition education via health fairs, schools, and other special requests.--MINIMUM EDUCATION-- * Bachelor's degree in nutrition or health education--REQUIRED LICENSURE/CERTIFICATIONS-- * Registered Dietitian with the Academy of Nutrition and Dietetics- * Licensed Dietitian in the State of Illinois- * IDPH Food Service Safety Certification OR Food Handler's Training and Certification- * CMS provider number--
