New Hope Pittsburg is looking for Direct Support Staff with the primary responsibility for coordinating consumer’s health care services; supervision of assigned cases and staff. New Hope Service, located in Pittsburg and Valley Center, Kansas, provides developmentally disabled clients with the services and support they need to empower them to live independently within their community. Medicalodges has developed, in New Hope Service, the expertise and highly trained personnel that are necessary to help people with developmental disabilities move into residential settings and personal homes and away from institutions.
JOB RESPONSIBILITIES:
- Responsible for assessing of consumer health care needs according to diagnosis, capabilities, support systems and Person Center Plan (PCP).
- Directing, supervising and evaluating the work performance of Nursing staff, Assistant Health Assistant, Shift Supervisors, House Managers and direct support staff in regard to consumer’s health care needs.
- Coordinating consumer’s medical/behavioral needs from admission, transfers and discharges.
- · Coordinating with Case Mangers and community resources to provide equipment and supplies to meet the medical/behavioral needs of the client.
- Coordinate and assist, as needed, the Nursing staff, the Assistant Health Assistant, Shift Supervisors, House Managers and Direct Support Staff to carry out the medical/behavioral portion of the consumer’s Person Center Plan (PCP).
- Assisting in education/training of Nursing staff, Health Services Assistant, Shift Supervisors, House Manager and direct support staff to the medical and psychological needs of the consumers.
- Obtaining/writing/interpreting doctor’s orders for Nursing staff, Health Services Assistant, Shift Supervisors, House Manager and direct support staff..
- Accurate and timely submission of all required documentation.
- Teaching/demonstrating consumer health care procedures, medication setup and/or administration to all staff/consumer/family as needed.
- Reviewing and updating monthly MAR’s for distribution by the House Manager..
- Responsible that medications ordered is available for consumers at all times.
- Responsible to review that correct medication has been delivered for monthly medication turn around.
- Monitor and assist when needed in the transporting of consumers for medically/behavioral related appointments.
- Responsible that consumers and staff are current on their TB vaccinations..
- Review, document and follow-up on all Unusual Behavior/Incident, Seizure, Skin reports and medication error(s) report in a timely manner and in compliance with all regulations.
- Observing and reporting immediately any changes in consumer’s physical condition to Physician, Nursing staff, Case Manager, Program Director and New Hope Director..
JOB REQUIREMENTS:
- Valid Kansas Nursing license.
- Is a graduate of an approved school of professional nursing.
- Preferred medical experience working with the ID/DD Consumer.
