DESCRIPTION OF WORK
The candidate shall provide services in conjunction with physicians, nurse practitioners, physician assistants, registered and licensed practical nurses, and support personnel. The scope of services provided in military treatment facilities cover the same range of services as those that are provided in civilian treatment facilities of similar size and scope of services. The work will be at the 51st Medical Group, Osan Air Base, South Korea.
DESCRIPTION OF EXPERIENCE REQUIRED
The candidate shall have evidence of at least two (2) years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years as a LVN/LPN.
DESCRIPTION OF LICENSING REQUIRED
The candidate shall have their current, active, valid, unrestricted license (with no limitations, stipulations or pending adverse actions) to practice as a LVN/ LPN from any US jurisdiction. License must be recognized and accepted by the State of practice in accordance with governing regulations.

- Performs administrative quality reviews of Duty Limiting Conditions (DLC), physical examinations, profiles, and appropriate clearances before these documents are forwarded/leave the MTF.
- Functions as the MTF’s key focal point and functional expert for the analysis, interpretation, and application of medical standards to include waiver actions.
- Represents medical standards element with a variety of installation and functional area organizations.
- Exercises supervisory personnel management responsibilities.
- Works with service members and/or veterans and their families who are experiencing a wide range of complicated mental, emotional, behavioral, physical, psychosocial, and environmental problems.
- Ensures all records are returned to the appropriate location as defined by the MTF within a timely manner.
- Working knowledge of computer applications/software to include Microsoft Office programs, MS Outlook (email), and internet familiarity is required.
- Must demonstrate strong ability to communicate in English effectively both orally and written.
- Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and potentially agitated.
- Must demonstrate ability to apply critical thinking skills and expertise in resolving complicated healthcare, social, interpersonal and financial patient situations.
- Must possess organization, problem-solving and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.
- Must have knowledge and skills to effectively apply LVN/LPN functions: a) Assessment: Identification of patients that need special medical and education needs/management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care; b) Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care; c) Facilitation: Care coordination and communication among all involved parties; d) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care coordination is received.
- Knowledge of overall medical service and individual medical treatment facility organization, management, mission, purpose, programs, related requirements in various administrative areas, philosophy, and objectives on a practical level, as well as an understanding of the JCAHO, AAAHC, and HIS standards and AF and DoD instructions.
- Knowledge of safety, security, and personnel management regulations, practices, and procedures and planning, organizing, and directing the function and staff of a small organization or element.
- Ability to provide and coordinate detailed consultation services and advice to, and gather information from, health care professionals, paraprofessional personnel and non-medical personnel.
- Ability to analyze, evaluate, reconcile contradictory requirements in, and draw conclusions from multiple sources of medical and technical information and analyze, interpret, and apply medical standards to general military population, aviators, and other special operational duty personnel as well as the different functions and motivations of the employees or groups involved.
- Ability to communicate orally and in writing in order to draft high level policy memorandums, present briefings, recommendations, and conclusions to health care professionals and senior management.
- Applies a comprehensive knowledge of rules, regulations, and policies related to the administrative and technical requirements necessary to coordinate, and carry out the administrative operations of the element.

- Ability to check in patients for appointments, including obtaining vital signs, check in and basic history.
- Must maintain a level of productivity and quality consistent with: complexity of the assignment; facility policies and guidelines; established principles, ethics and standards of practice of professional social work, Health Services Inspection (HSI); and other applicable DoD and service specific guidance’s and policies.

- Completes medical record documentation and coding and designated tracking logs and data reporting as required by local MTF/AF/DoD instructions, policies and guidance.
- Completes all required electronic medical record training, MTF-specific orientation and LVN/LPN training programs.
