    Title of Immediate supervisor: Clinical Supervisor/Case Manager

    General Duties

    Responsible for the continuous improvement of the total quality care as it related to speech therapy rendered to the patient/client.

    Essential Functions

    • Responsible for delivering speech therapy care to patient/client, as directed by the physician’s plan of treatment and interdisciplinary patient/client plan of  care.
    • Responsible for reporting on the condition of patient/client to the appropriate supervisor, and other staff members.
    • Responsible for performing speech and audiology assessment and procedures which require substantial specialized knowledge, judgment and skill based upon the principles of psychological, biological, physical and social sciences. Responsible for making decisions based on individual educational preparation and experience as a speech therapist.
    • Responsible for coordinating the care of all assigned patients/clients to assess and identify needs and review the agency’s range of services resulting in achievement of expected goals, active participation in case conferences and making suggestions to appropriate supervisors. 
    • Complete other assignments as requested and assigned.
    • May have access to personal health information(“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.
    1. Assists the physician in evaluating the level of function, helps develop the plan of care(revising it as necessary)
    2. Prepares Clinical and progress notes
    3. Advises and consults with the family and other agency personnel
    4. Participates in In-Service Programs.

    Working Conditions & Physical Efforts Requirements

    • Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
    • Physical activities will include, walking, sitting, stooping, and standing and minimal to a maximum lifting of patients and the turning of patients.
    • Reaching: Extending the hands and arms in any direction.
    • Handling: seizing, holding, grasping, turning, or otherwise working with the hand or hands.
    • Fingering: Picking, pinching, or otherwise working with the fingers primarily (rather than with the whole hand or arm as in handling).
    • Feeling: Perceiving such attributes of objects and materials as size, shape, temperature, or texture, by means of receptors in the skin, particularly those of the finger tips.
    • Talking: Expressing or exchanging ideas by means of the spoken word.
    • Hearing: Perceiving the nature of sounds by the ear.
    • Acuity, far: Clarity of vision at 20 feet or more.
    • Acuity, near: Clarity of vision at 20 inches or less.
    • Depth perception: 3-Dimensional vision. The ability to judge distance and space relationships so as to see objects where they actually are.
    • Field of vision: The area that can be seen up and down or to the right or left while the eyes are fixed at a given point.
    • Accomodation: Adjustment of the lens of the eye to bring an object into sharp focus. This item is especially important when doing near- point  work at varying distances from the eye.
    • Color vision: The ability to identify  and distinguish colors. 
    • The ability to access patients’ homes which may not be routinely wheelchair is required.  Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.
    • The ability to communicate both verbally and in writing is required as frequent communication by telephone and in writing in English is required.
    • Work schedule requires flexibility.

    Company Information

    Has access to all client medical records which may be discussed with the Registered Nurse and the Nurse Supervisor.

    Minimum Education & Experience Requirements

    • Masters prepared graduate with a degree in Speech Pathology/Audiology, from a university approved by the American Speech- Language Hearing Association.
    • State licensure may be required where applicable.
    • A teaching certificate may be required to practice in a public school.
    • At least one(1) year of Speech/Language Pathology experience within the past five(5) years and defined by the contracting employer.
    • Evidence of current BAsic Life Support(BLS) training for infants, children and adults.

    Knowledge, Skills & Abilities Required

    • Must be a graduate of a Speech Therapy school approved by the CAA (Council on Academic Accreditation) for Auditory and Speech-language Therapists or equivalent experience to the Job description.
    • Must be unencumbered license and registered by the State as a speech therapist.
    • One or more years of experience in the community/home health agency or in a hospital setting is preferred.
    • Must have a current CPR certification.
    • Must have knowledge of Medicare and Medicaid guidelines.
    • Must have a working knowledge of home health care and the principles and techniques of professional physical therapy techniques and required documentation that pertains to it.
    • Should be skilful in the organization and in the principles of time management and have knowledge of management processes.
    • Must be able to contribute to the quality of care being rendered through constructive communication with nursing managers and staff.
    • Must pass a criminal background check.
    • Must have working experience in EMR systems.
    • Must have a valid driver’s license, valid auto insurance and be willing and able to drive to patient’s residences.
    • Must have a good driving record.
    • Must be able to work independently as well as with team members with minimal supervision.
