Medical Records Clerk

The Medical Records Clerk is responsible for creating, maintaining, closing patient charts and communication of information related to the treatment of patients under the care of Crossroads Hospice & Palliative Care.

Job Duties

  • Creates open charts for all new admissions, files clinical documentation daily, records documents in QAPI log form, processes related documents as received and ensures charting is secured
  • Processing of all forms to be mailed/faxed related to the planning of treatment and medical records
  • Answers incoming calls and directs to appropriate provider as needed
  • Assists and Consults with Bereavement Coordinator in preparation of bereavement file and other related bereavement issues as needed
  • Knowledge of insurance guidelines, including HMO/PPO, Medicare, and state Medicaid
  • Ensuring clinical forms are available for employee use
  • Prepares documents needed or signatures for scheduled IDG meetings and participates in regularly scheduled meetings
  • Maintains sign-in/sign out in the system for the removal of records by clinical staff
  • Works in conjunction with Medical Records Coordinator to complete the day to day responsibilities within the medical records department as dictated
  • Communicates missing documents, deficient documentation and any other issues identified during the receipt, filing and tracking of clinical documentation to the appropriate team leader, emotional support team leader, clinical director and/or executive director immediately
  • Any other duties assigned by the Clinical Director, Executive Director and/or Carrefour personnel

Education and Experience Required

  • High school diploma plus completion of technical school related to medical records, record keeping or similar field
  • Knowledge of business and accounting processes usually obtained from an associate's degree, with a degree in Business Administration, Accounting, or Health Care Administration preferred
  • Minimum of 1 to 3 years of experience in a medical office setting preferably in Medical Records
  • Completion of technical school related to medical records, record keeping or similar field preferred
  • Customer service skills for interacting with patients regarding medical claims and payments, including communicating with patients and family members of diverse ages and backgrounds
  • Problem-solving skills to resolve arising issues and ability to multitask
  • Must work well with others

  • Mon to Fri 8:30 AM to 5:00 PM
