    COMPENSATION:
    - $65,000 to $80,000 Base Salary, dependent upon experience

    LOCATION:
    - Hudson County, New Jersey
    - greater Jersey City area

    The new RN will work between 2 or 3 practices in Hudson County, spending approximately 4 months at a time in each location to manage and help implement the Medicare patient management program, then conducting rounds, audits, quality checks, etc. on an ongoing basis.  The practice are located very near to each other.

Providers tailor care plans to each individual, integrating preventative medicine, chronic disease management, behavioral health, medication management, and urgent care services. They provide seamless support to ensure that their vulnerable geriatric patient base never falls through the cracks of the system, and that unnecessary hospitalizations are avoided. They operate in dozens of New Jersey communities and several New York locations, offering in-home services, remote options, clinic-and-office-based services, and on-demand urgent care. Their provider staff of physicians, nurse practitioners, physician assistants, registered nurses, LPNs, and medical assistants work in collaborative settings with full support and resources in every location.


    POSITION DESCRIPTION:
    The new RN will provide a variety of clinical and administrative tasks to support the work of physicians and other healthcare professionals who manage chronically ill patients in an outpatient setting.

    This position assists the physicians in conducting annual wellness visits, identifying preventive measures, creating care plans for chronic conditions, and conducting advance care planning. 

    The RN will be responsible for supervising the implementation of the care plans and behavioral coaching by other clinical staff.

    The RN is also responsible for working with patients and providers to determine the type of care that is needed, individualized to each patient.  Each patient’s case is reviewed thoroughly in order to determine how it helps improve the patient’s quality of life and improve quality metrics.

    In creating a plan, patient ability and needs are considered. Within the plan, the RN may include treatment methods, medication, nutrition, and exercises that need to be carried out by the patient.

    Other duties include, but are not limited to:

    - Assess patient needs and develop a care plan.
    - Work with all providers to deliver the best quality care for patients.
    - Fulfill patient care responsibilities as assigned by providers and supervising physician
    - Collect patient intakes.
    - Assist with scheduling as needed.
    - Prepare and educate patients for their visits and/or procedures.
    - Relay instructions to patients/families.
    - Ensure that all patient records are updated and accurate in the EHR.


    REQUIREMENTS:
    - active New Jersey RN license
    - proficient and comfortable with technology
    - must speak Spanish fluently
