Founded in 2014, this unique organization works with private practices to take over management of their Medicare patients. The organization and its providers work specifically with this unique population to ensure they are fully utilizing all of the available resources to optimize their health and well-being. They believe strongly that primary care should be preventative and proactive - they take a holistic approach that expands the relationship between provider and patient. Providers tailor care plans to each individual, integrating preventative medicine, chronic disease management, behavioral health, medication management, and urgent care services. They provide seamless support to ensure that their vulnerable geriatric patient base never falls through the cracks of the system, and that unnecessary hospitalizations are avoided. They currently operate in dozens of New Jersey communities and several New York locations, offering in-home services, remote options, clinic-and-office-based services, and on-demand urgent care. Their provider staff of physicians, nurse practitioners, and physician assistants work in collaborative settings with full support and resources in every location.
This is a full-time position working within a private practice in Leonia, New Jersey. The new team member will be employed by the company described above, but will work at a contracted private practice.
Providers see an average of 8 to 12 patients per day in an outpatient setting. All patients are geriatric Medicare recipients.
The new Behavioral Healthcare Manager (BHM) will be the designated behavioral resource for their team. They must have formal education and/or specialty training in behavioral health.
The BHM will work directly under patients' primary care providers, collaborating as an integral member of the care team.
The practice uses an EHR.

- appropriate education/licensure is required based upon type of provider
- formal education and/or specialty training in behavioral health
- must speak Korean fluently
