    COMPENSATION:
    - $170,000 to $200,000 Annual Base Salary, negotiable dependent upon experience

    - Greater Akron Area

    During the interview process, the hiring managers will work with the physician to determine appropriate, convenient practice site(s), as patients are seen in ALFs, ILFs, and private residences.  Territories are built around providers’ home locations to ensure minimal drive time.



    COMPANY PROFILE:
    Geriatric primary care practice serving assisted and independent living facilities (ALFs/ILFs), as well as private residences, throughout Ohio and Northern Kentucky.  The practice was founded and is led by a physician with 15+ years of experience in the home-based primary care field.  They currently employ a team of 15 talented physicians, nurse practitioners, and physician assistants, who are supported by dedicated clinical and administrative team members.  They are hiring due to growth.



    POSITION DESCRIPTION:
    Serve as a primary care provider for a dedicated base of geriatric patients.

    The clinical focus is on preventative medicine, chronic disease management, and routine geriatric primary care.

    See your patients as often as you deem necessary, which is typically 1 visit per month.

    Travelling to 1 or 2 facilities each day (with an occasional house call), see an average of 10 - 12 patients per day.  Territories are built around the provider’s home location to ensure minimal travel time.

    The patient breakdown is:

    - 70% ALFs/ILFs
    - 30% private residences

    Utilize a highly-supported mobile electronic health record (GEHRIMED) and communicate with your team via phone. All orders can be placed through the EHR and will be carried out by your office-based coordinator.

    Providers share on-call responsibilities.  The current rotation is 1 week every 4 to 5 months and will decrease as providers are added to the practice.  On-call is phone triage only and volume is very light.



    REQUIREMENTS:
    - Ohio medical license
    - DEA
    - Board Certification or Eligibility, relevant field
