    - $180,000 to $200,000 Annual Income, dependent upon experience

    LOCATION:
    - Greater Surprise Area
    - This practice sees patients in a clinic in Surprise, as well as in assisted and independent living facilities in the surrounding area.


    COMPANY PROFILE:
    This multi-faceted healthcare organization has been operating in the Phoenix area for more than 20 years. They are currently hiring in their internal medicine / primary care area, which sees patients in a clinic and via primary care to ALFs and ILFs. Each area of the practice has its own Medical Director and the practice's physicians also have the opportunity to serve as Medical Directors at contracted facilities. The practice boasts a provider staff of more than 15 physicians, PAs, and NPs, who are supported by clinical assistants, administrative staff, and management.


    POSITION DESCRIPTION:
    The new physician will split their time between the Surprise primary care clinic and nearby facilities (ALFs / ILFs). The exact split between clinic and facility work is negotiable.

    Typical duties include, but are not limited to:

    - routine primary care on a geriatric and internal medicine population
    - minor walk-in / acute care
    - chronic disease management
    - annual wellness visits
    - well-woman examinations

    The clinic patient base is 80% geriatric and 20% internal medicine. In facilities, it's 100% geriatric.

    See 20 to 25 patients per day. If visiting more than 1 facility in a day, this number is reduced according to the required travel.

    Medical assistants are available to room/prep patients and assist providers as needed, including in facility settings, if desired.

    The clinic uses the PracticeFusion EHR.

    There is a minor on-call component. On average, each physician fields 1 or 2 phone calls per week. The practice utilizes a phone triage service after hours and the Medical Directors are always available as back-ups, should the patient's PCP not be available.


    REQUIREMENTS:
    - Arizona medical license
    - DEA
    - board certification/eligibility is preferred but not required
    - experience with well-woman exams
