- $85,000 to $100,000 Annual Salary

LOCATIONS:
Colorado Springs, Colorado
During the interview process, the hiring managers will discuss available practice sites. Most patients are in group settings, such as ALFs and ILFs, meaning minimal travel during the day. Colorado Springs is a new territory for the practice and, for the first few months, driving distance will be greater, until other providers are hired and the territories are divided up based on home location.

The leadership team behind this innovative, compassionate private practice have been pioneers in the market of home-based primary care for more than 20 years, delivering care to the most vulnerable patient populations in Colorado, in the comfort of their own homes and facility settings. Their service offerings include primary care, palliative care, chronic disease management, dementia care, transitional care, and specialty care (through their vast network of referral partners). They employ a dedicated team of physicians, nurse practitioner, and physician assistants, as well as talented management and administrative support staff. They are currently hiring due to growth.
POSITION DESCRIPTION:
Serve as a primary care provider for a dedicated base of geriatric patients.
See your patients as often as you deem necessary, which is typically 1 visit per month.
See an average of 8 to 10 patients per day. Initially, to accommodate the extra drive time in the new territory, the provider will see fewer patients to ensure days are kept to approximately 8 hours total with driving and patient visits.
Patients are scheduled by the office team, ensuring travel time is kept to a minimum and routes are efficient. Many patients are in facility settings (ALFs/ILFs), allowing for multiple patients to be seen at a single location. Some patients are in private homes, being seen in a traditional house call capacity.
Providers begin and end their days at their own homes, requiring minimal visits to the office. Once a month, the provider will attend a staff meeting in Wheat Ridge.
The new provider will focus on:
- preventative medicine
- chronic disease management
- geriatric primary care
- palliative care
- sick visits
- dementia care
- transitional care
- specialist referrals
The practice uses the eClinicalWorks EHR.
While providers practice autonomously in the field, they are supported by a dedicated, highly-trained office team of schedulers, clinical coordinators, and personnel who answer phone calls, place orders, and assist however they can remotely from the office.
There are no weekend, evening, or on-call responsibilities, allowing for excellent work/life balance.
REQUIREMENTS:
- Colorado NP or PA license
- DEA
