Founded in 2014, this unique organization works with private practices to take over management of their Medicare patients. The organization and its providers work specifically with this unique population to ensure they are fully utilizing all of the available resources to optimize their health and well-being. They believe strongly that primary care should be preventative and proactive - they take a holistic approach that expands the relationship between provider and patient. Providers tailor care plans to each individual, integrating preventative medicine, chronic disease management, behavioral health, medication management, and urgent care services. They provide seamless support to ensure that their vulnerable geriatric patient base never falls through the cracks of the system, and that unnecessary hospitalizations are avoided. They currently operate in dozens of New Jersey communities and several New York locations, offering in-home services, remote options, clinic-and-office-based services, and on-demand urgent care. Their provider staff of physicians, nurse practitioners, and physician assistants work in collaborative settings with full support and resources in every location.
This is a full-time position working within a private practice in Leonia, New Jersey. The new MA will be employed by the company described above, but will work at a contracted private practice.
The patient base is largely Korean-speaking and the new MA must also be fluent.
Providers see an average of 8 to 12 patients per day in an outpatient setting. All patients are geriatric Medicare recipients.
The new MA will assist physicians, NPs, and PAs who are creating care plans, ensuring that patients are kept at low risk for hospitalization and utilizing every resource available to ensure their health and overall well-being.
Typical duties will include rooming patients, taking vitals, performing blood draws, and assisting providers as needed.
The practice uses an EHR.
- completion of an accredited Medical Assistant program
- must speak Korean fluently
