COMPANY PROFILE:
The hiring organization is a new Medicare Advantage Health Plan exclusively available to long-term skilled nursing facility (SNF) residents in Oklahoma City, Tulsa, and the surrounding counties. Eligible residents will be able to enroll in the plan in mid-October of 2019 and coverage begins on January 1, 2020. The company is a SNF owner/private equity partnership plan that currently operates in two other states and will be operating in five states total by the end of the year. They operate on a patient-centered nurse practitioner clinical model in which the nurse practitioners partner with the residents’ primary care and nursing teams, focusing on improving quality and appropriateness of care and ensuring the residents’ goals of care are achieved.
POSITION DESCRIPTION:
The Clinical Operations Field Manager (COFM) is responsible for overseeing the day-to-day activities conducted by the clinical delivery providers working in the network SNFs. This includes, but is not limited to:
- managing local customer service issues
- engaging SNFs in the health plan model of care
- educating/training
- monitoring care delivery provider performance
- overseeing the process of care delivery in contracted SNFs
- collaborating with the provider network to enhance and improve the overall quality of care
The COFM works with the network manager and contracted/delegated care delivery and support entities to ensure clinical quality and affordability initiatives are executed and targeted outcomes are achieved. Specific responsibilities include:
- conduct rounds at each contracted SNF on a regular basis to determine if providers are delivering the health plan model of care and meeting quality and regulatory standards
- conduct periodic review meetings with contracted SNFs to review clinical and quality results of residents’ care
- discuss successes and opportunities for improvement
- promote open communication
- review daily clinical performance metrics and take action if not on target
- assist with readiness assessments, education, and training
- perform oversight audits
- perform medical record documentation reviews for contracted providers
This position reports to the Director of Quality and Clinical Operations and directly supervises/manages/coaches employed and contracted nurse practitioners (no more than 12 NPs) to include training, performance management, hospitalization reviews, and more.
REQUIREMENTS:
- Oklahoma NP license
- active DEA
- appropriate certification; adult-gerontology preferred
- previous experience in SNF settings
- previous leadership experience
- previous experience in clinical quality programs, utilization management, etc.
