Nurse Practitioner Resident- Family

Reports to: Chief Medical Officer, Collaborative Physicians and Residency Director

Responsibilities and Duties:

Provide primary care to patients under supervision of a collaborative physician and follow established medical protocols. Collaborates with physician to formulate and review medical protocols. Obtains complete or interval history of the patient, identifies patient risk factors, identifies health hazards in the patient's environment and provides health education. Performs appropriate physical exams as indicated, obtains appropriate laboratory specimens and analyzes clinical data for appropriate diagnosis and treatment. Assures treatment and/or plan of care are consistent with established protocols as well as patient preferences, and assures treatment is revised as necessary according to patient outcome. Documents all pertinent information accurately, legibly, and in a timely manner. Assures appropriate consent forms are signed and dated. Refers patients to appropriate services which exceed services of the clinic or scope of practice of the practitioner. Assists in assuring that adequate patient health records are maintained and transferred as required when patients are referred. Seeks out community resources to meet or assist with the patient's needs. Assists medical director in supervising other clinic staff. 

Identifies and considers the cultural patterns, economic status and patient resources in plan of care.

Participates in studies, audits, and surveys as requested. Assists with home visits as needed.

Performs a daily review of the patients who have no-showed for appointments on the day prior to ensure the appropriate follow-up is rescheduled. Performs a daily review of labs that have resulted as well as images and referral notes that have been sent to ensure the proper documentation in the patient’s chart as well as appropriate follow-up has been scheduled. Participates in taking night phone call in rotation with other clinic providers.

Participates in Emergency Room (ER) rotations. Patient care will be in collaboration with the ER physician. ER rotations will be 6 hours in duration, one weekend shift per month and located at Bibb Medical Center ER in Centreville, Alabama . Travels to Centreville or other Cahaba sites for workshops when available (casting, splinting, suturing, etc.). Completes online training modules when indicated.

Qualifications:
- Graduation from an accredited school of nursing and nurse practitioner training program within the past 18 months prior to October 1st, 2019
- Licensure with the state of Alabama as a nurse practitioner and registered nurse
- Certification as a nurse practitioner in Family Practice
