Licensed Social Worker - West End Location

Reports to: Chief Medical Officer

Responsibilities and Duties:
  • Coordinate transportation for CMCF patients
  • Coordinate patient assistance program, drug assistance programs and access other providers' sliding fee and charity care programs
  • Offering counseling sessions to referred mental health patients with communication back to the referring primary care physician on a scheduled basis
  • Develop and manage screening process for front office staff to identify patients who may eligible for the CMCF sliding fee discount program, as well as patients who may be eligible for insurance (Medicaid, Medicare) and assist patients in walking through the enrollment process
  • Assist patients in applying for CMCF's sliding fee discount program
  • Assisting providers during clinic / patient care hours with ongoing social issues affecting the patients’ well-being including financial problems, housing issues, and coordination with the County and State in dealing with potential issues of abuse or misuse
  • Assisting clinic and staff and patients with problems in obtaining needed medications or health care services
  • Provides crisis intervention services to patients in need
  • Refers clients to appropriate community resources and advocates for patients by contacting these agencies when necessary on the patient’s behalf
  • Assists with patient care planning including helping providers create individualized care plans for chronic disease management 
  • Assists with patient education and helping create patient education forms and action plans for patients to take home following their appointments
  • Complete application process to become  a Certified Application Counselor and Medicaid enrollment counselor.
  • Perform Insurance Outreach and Enrollment Assistance 
  • Provide comprehensive medical case management services to ensure that patients have access to affordable healthcare, public assistance programs, and community resources
  • Screen patients for mental and behavioral disorders and refer patients, as necessary, to appropriate providers
  • Perform outreach in Jefferson County to identify and assist residents in need of healthcare and social services

  • Bachelors or Masters in Social Work
  • At least three years experience working as a social worker in Jefferson County
  • Active Alabama social work license
