Charleston Internal Medicine is looking for a physician that will provide medical services including direct patient care, medical record documentation and follow up care in an outpatient setting.
In addition the physician will help navigate patients through the care system and process assuring that they have the means and ability to overcome barriers (financial, transportation, language, literacy, social) and receive needed/recommended care in a timely, organized, and comprehensive manner- eliminating no-shows and loss to follow-up while also maintaining confidentiality of all information.
Requirements:
- Graduated from an accredited MD or DO program
- Current license to practice medicine in the State of WV
- Board certified or board eligible with preference given to board certified
- Highly organized and well-developed oral and written communication skills
- Demonstrate sound judgment, decision making and problem solving skills
- Understanding of current recognized standards of care within the scope of licensure
- Thorough knowledge of applicable State and Federal laws and regulations
- Valid DEA number
- Current CPR certification
- Valid driver’s license
- Ability to establish and maintain effective working relationships with patients, families, medical staff and co-workers
- Support practice missions and goals
Reports to: Lead Physician
Essential Duties and Responsibilities:
- Perform comprehensive and interval history and physician exams of patients with acute and chronic health problems
- Identify differential diagnoses, assess the severity of a patient’s illness and establish priority of care for the presenting problem.
- Development of a treatment plan and appropriate follow up care
- Participate in value based reimbursement programs that are supported by the practice.
- Communicate pertinent patient information with other members of the patient’s care team
- Educate patient about their diagnosis and treatment plan
- Order, interpret and evaluate appropriate diagnostic tests
- Obtain informed consent for any proposed procedure
- Write prescriptions within the scope of licensure
- Document patient encounters appropriately and timely (within 24 hours of service) within the guidelines established by CIM
- Analyze new knowledge gained from conferences, workshops, professional literature or other training and assimilate this knowledge into the clinical practice
- Facilitate patient medication management based upon standing orders and protocols
- Support patient self-management of disease and behavior modification interventions with patients
- Promote clear communication amongst care team by ensuring awareness regarding patient care plans
- Assist in the education and training of staff and precept students
- Seek appropriate consultation with other CIM providers when needed
- Knowledge of and compliance with emergency plan and OSHA standards
- Participate in provider meetings
- Participate in ongoing professional development activities
- Participate in after-hours call on a rotating basis
- Maintain appropriate dress standards
- Maintain a clean and organized work space
- Assist in keeping the total CIM facility clean for both patients and co-workers
- Work with other members of the CIM team in a pleasant and courteous manner
